1. Build a Customizable PRP Informed Consent Form
Start with an evidence-informed PRP consent framework, then tailor the procedure, screening workflow, financial language, and form sections to match your practice and patient. Common starting options are already selected and remain editable.
PNG, JPG, or WebP. Your logo appears on the printable form and electronic patient screen.
Upload a signature image for the signed-in clinician. It is saved only as a clinic-level default and is applied only after the clinician chooses Use Saved Signature for a specific consent.
2. Customize Sections
Core consent sections start ON. Green and right means included. Black and left means not included.
