PRP Pre- and Post-Procedure Guidelines
What clinicians are doing, why they are doing it, and what the evidence actually supports for medication management, patient preparation, post-procedure care, and indication-specific recovery guidance.
PRP post-procedure instructions and pre-procedure recommendations vary substantially between physicians, institutions, published clinical studies, and expert consensus statements. That variation does not necessarily mean that one protocol is correct and the others are wrong. In many cases, clinicians are attempting to protect different parts of the PRP biologic response using evidence that remains incomplete. [1]
Many pre-procedure considerations, including medication review, platelet-active drugs, hydration, alcohol, and blood-draw preparation, apply broadly across PRP practice. Post-procedure recommendations are more indication-specific and should be adapted to the treatment area, procedure, and clinical objective.

There is no universally accepted NSAID or activity protocol surrounding PRP. Current recommendations from academic centers and expert groups range from short pharmacologic washout periods to several weeks of medication avoidance.
The variation itself is clinically informative. It demonstrates that current protocols incorporate more than the duration of measurable platelet aggregation inhibition.

A physician using a 7-, 10-, or 14-day NSAID restriction is not necessarily applying outdated pharmacology. That clinician may be attempting to protect platelet activation, growth-factor release, and the broader PRP biologic response rather than simply waiting for platelet aggregation to normalize.
How Long Should Patients Avoid NSAIDs Before PRP?
Most non-aspirin NSAIDs inhibit platelet cyclooxygenase reversibly. Once sufficient drug has cleared, platelet aggregation can recover without waiting for an entirely new platelet population.
A 2024 review of 25 studies found approximately 6–12 hours of measurable platelet inhibition with ibuprofen and diclofenac, recovery by approximately 24 hours after indomethacin, and inhibition lasting at least 24 hours and possibly 48 hours with naproxen. [7]

Platelet aggregation is only one measurable aspect of platelet biology. PRP treatment also depends on platelet activation, granule release, growth factors, cytokines, extracellular vesicles, and interactions with the target tissue. Normalization of one aggregation assay does not establish normalization of every biologically relevant PRP function.
Why do many protocols still use 7–14 days?
The French expert consensus cited a human study in which naproxen altered concentrations of selected PRP growth factors, including PDGF-AA and PDGF-AB, with effects reported until approximately one week after naproxen discontinuation. The experts ultimately considered a one-week NSAID interruption before and after PRP reasonable, while emphasizing the absence of comparative clinical outcome data. [2]
The more recent ESSKA-ORBIT European knee OA consensus took an even more conservative position. It recommends avoiding NSAIDs for two weeks before PRP because of potential effects on platelets and in-vivo growth-factor release. This recommendation received Grade C evidence rather than high-level Grade A support. [3]
For an elective musculoskeletal PRP procedure, a 7-day pre-procedure avoidance of nonselective NSAIDs is a reasonable conservative starting protocol when medically appropriate.
A 14-day interval is also defensible for clinicians following more conservative institutional or European knee OA recommendations, although current evidence has not demonstrated that 14 days produces superior clinical outcomes compared with 7 days or shorter medication-specific intervals.
The shorter pharmacologic intervals remain useful information. They help clinicians understand that a seven-day recommendation should be described as a PRP protocol choice rather than the time required for ibuprofen itself to stop inhibiting platelet aggregation.
Should Patients Stop Aspirin or Antiplatelet Therapy Before PRP?
Aspirin irreversibly inhibits platelet cyclooxygenase. Unlike most conventional NSAIDs, recovery therefore depends substantially on the introduction of newly produced platelets rather than simple drug clearance.
That pharmacology provides one historical explanation for longer peri-procedural platelet-management intervals. It does not, however, establish that every patient receiving aspirin should discontinue it before PRP.
Patients may be taking aspirin or another antiplatelet medication for secondary cardiovascular or cerebrovascular prevention. In these patients, the potential PRP benefit of stopping therapy must be weighed against a very different level of medical risk than stopping occasional OTC ibuprofen.
The French knee OA consensus concluded that antiplatelet therapy should not automatically contraindicate PRP and generally recommended against interruption because of potential cardiovascular complications. [2]
Do not routinely instruct patients to discontinue prescribed aspirin, clopidogrel, or other antiplatelet therapy solely because PRP is planned.
Determine the indication first. If temporary interruption is being considered, involve the clinician managing the antiplatelet therapy when appropriate and weigh the medical risk against the uncertain magnitude of any PRP-specific benefit.

How Should Patients Prepare Before a PRP Blood Draw?
Medication timing receives the most attention, but blood collection occurs within a broader physiologic environment. Hydration, meals, alcohol, smoking, recent illness, and medications can all be clinically relevant. The strength of PRP-specific evidence, however, differs considerably among them.

Before blood collection, one concern is whether NSAIDs alter the platelet product being prepared. After injection, the question becomes broader: could suppressing cyclooxygenase-dependent inflammatory signaling alter the early tissue response PRP is intended to initiate?
Tissue repair is not simply an anti-inflammatory process. Early inflammatory signaling participates in recruitment, cellular communication, vascular responses, matrix turnover, and subsequent remodeling. This provides a biologically plausible reason to avoid aggressive anti-inflammatory therapy during an early healing phase, even after the injected platelets have already activated.
That rationale helps explain why post-PRP restrictions are often longer than medication-specific pre-draw pharmacologic washout periods.
Current clinical recommendations remain diverse. ESSKA-ORBIT recommends one week after knee PRP. WashU uses two weeks. McGovern uses two weeks. The RNOH tendon protocol extends the restriction to six weeks. [3] [4] [5] [6]
For uncomplicated musculoskeletal PRP, approximately 1–2 weeks of nonselective NSAID avoidance after treatment is a reasonable default when medically appropriate.
For chronic tendon PRP, approximately 2 weeks provides a practical conservative starting point and is consistent with contemporary expert practice.
Longer restrictions may be selected for specific tendon protocols, but physicians should recognize that several-week restrictions remain driven more by biologic rationale and expert practice than comparative human outcome trials.
When Should Rehabilitation Begin After Tendon PRP?
Tendon rehabilitation is one of the most clinically useful areas of the emerging literature. The 2026 international Delphi consensus involving 23 experts reached agreement on rehabilitation timing, progressive loading, pain thresholds, and return to activity after intratendinous PRP. [8]
Institutional protocols frequently prescribe an initial protected period followed by rehabilitation. WashU begins rehabilitation around 2 weeks, while the RNOH tendon pathway also avoids strenuous tendon use for 2 weeks before physiotherapy. [4] [6]
The 2026 consensus supports an earlier transition toward structured tendon loading, recommending that rehabilitation begin approximately 5–10 days after injection. [8]
Relative protection
Avoid provocative or high-load activity during the early symptomatic phase. Immobilization should not be automatic simply because PRP was administered. Protection should reflect the tendon, structural pathology, injection technique, pain, and risk.
Begin controlled rehabilitation
International experts reached strong consensus that tendon rehabilitation should begin relatively quickly, approximately 5–10 days after intratendinous PRP for chronic tendinopathy. [8]
Use sub-painful isometrics
A useful clinical threshold is pain ≤3/10 during the exercise session and for the subsequent 24 hours. [8]
Add dynamic loading
Progress toward concentric and/or eccentric loading while maintaining the same ≤3/10 pain response during exercise and through the subsequent 24-hour period. [8]
Reproduce real mechanical demand
Progress rehabilitation toward the specific loading patterns required by the patient's occupation, recreational activity, or sport. [8]
Use function rather than calendar alone
Gradual return to work or sport can be considered when the clinical examination demonstrates little or no pain. The consensus used VAS <3/10 as a practical threshold and did not consider imaging necessary solely for return clearance. [8]
When Can Patients Return to Activity After Joint PRP?
A knee joint and a pathologic tendon face very different mechanical environments. Applying the same post-procedure protection period to both simply because PRP was injected is difficult to justify.
In the 2024 systematic review of 187 knee OA PRP studies, only 35.3% included an activity restriction and the most common duration was one day. Return-to-activity instructions appeared in only 20.3% and were usually described as gradual or as tolerated. Only 16% reported a physical therapy protocol. [1]
After an uncomplicated intra-articular knee PRP injection, consider relative activity for the remainder of the procedure day and approximately 24–48 hours, while allowing routine activities of daily living as tolerated.
Gradually restore exercise according to joint irritability, effusion, pain, baseline OA severity, and the therapeutic objective. Prolonged unloading should have a reason beyond the fact that PRP was injected.
Should Patients Use Ice After a PRP Injection?
Some clinicians avoid ice because they do not want to suppress the early inflammatory response or potentially alter platelet activity. Others use brief cryotherapy as symptomatic treatment.
Current protocols demonstrate both approaches. WashU specifically recommends icing for 15–20 minutes every 2–4 hours during the first few days, and McGovern permits ice and compression for pain relief. [4] [5]
The 2026 tendon consensus specifically evaluated whether immediate ice might alter platelet activation and classified the recommendation as uncertain. [8]
Do not present “never use ice after PRP” as a demonstrated biologic requirement. A clinician may elect to minimize routine aggressive cryotherapy, particularly after tendon PRP, while still permitting limited symptom-directed cooling when appropriate.
